Remap CODE_REJECTED_ELSEWHERE to incoming rejected. CODE_REMOTE_CALL_DECLINE and CODE_REJECTED_ELSEWHERE are both explicitly documented as handling the "call rejected elsewhere" multiendpoint usecase. Yes, they're exactly the same.
