Fixed the logic for tethering provisioning re-evaluation
Previously we only re-evaluate provisioning for SIM swap case
The new logic covers both SIM swap case
(ABSENT->NOT_READY->UNKNOWN->READY->LOADED) and modem reset
case (NOT_READY->READY->LOADED)
