pan/bi: Pack BI_FMA ops This is our first instruction we've emitted, requiring us to pipe through registes/ports and various details from the IR. It's quite a bit of code, but overall I'm happy with this structure. With some tedium we should be able to emit the rest of the ALU ops this way, too.
